Membership Changes

We know that via Executive Order, Family Wellness will be closed at least through April 6th with reopen subject to the impact of COVID-19 in our community. Family Wellness will not run billing during a closure, and there is no action required on your behalf to avoid a charge. Once the length of the downtime has been determined, we will communicate a plan to reopen and address any billing concerns. We encourage our members to remain active during this time to help us accommodate membership related credits when we reopen. 

Use the links below to adjust your membership to best fit your needs. Once received, our member relations team will follow up with any additional instructions. Use the freeze my membership button below to submit for a membership hold if you know you are not going to be able to use your membership for a prolonged period of time. Use the cancel/downgrade my membership button below to downgrade to a different membership type or cancel your membership completely. All membership change requests must be submitted no later than the 15th of the month prior to the effective month.

COVID-19 Membership Freeze | Family Wellness is taking preventative measures to ensure the safety of our members and staff as our community learns more about COVID-19. However, if you are at an increased risk or are taking extra precaution please use this form is for placing your active membership on a hold for the month of April (for no cost). Placing your membership on freeze removes access for all persons listed on the membership. Access and billing for frozen memberships using this option will automatically start May 1st. All submission using this option will be accepted if completed prior to April 1st. Freezes for additional time or reasons should use our typical Freeze Form.
