Change Membership

The Membership Change Form allows members to make changes to their membership account. Members have the ability to cancel their membership completely, choose certain people on their membership to cancel (pertains to family memberships), or downgrade to a different membership type. All membership change requests must be submitted before the 15th of the month prior to the first of the month in which the change is effective to avoid future drafts from the account.

  • Membership Cancellation/Downgrade

    Use the options below to indicate whether you plan to cancel your membership completely or downgrade to a different membership type. Please Note: Upgrading to a more expensive membership type can only be done at Family Wellness.
  • If the current main member on the account is cancelling, new billing information will be needed once the downgrade has been processed. We will contact you to update the billing information prior to the effective cancellation date. If we are unable to attain your updated billing information, the entire account will be cancelled.
